What is Discharge to Assess (D2A)?
Discharge to Assess (D2A) is a pathway that allows patients to be discharged from the hospital when they are medically fit but still need further assessment and support to determine their long-term care needs. The goal is to provide a safe and supportive environment where patients can continue their recovery and receive the necessary assessments outside of the hospital setting

D2A Pathways
There are four main D2A pathways, each designed to meet different levels of care needs:
- Pathway 0: Discharge home or to the usual place of residence with no new or additional health and/or social care needs.
- Pathway 1: Discharge home or to the usual place of residence with new or additional health and/or social care needs.
- Pathway 2: Discharge to a community bed-based setting with dedicated recovery support. This pathway is for individuals who need short-term health and/or social care support to recover before returning home or moving to long-term care.
- Pathway 3: Discharge to a new residential or nursing home setting for individuals likely to need long-term residential or nursing home care
Assessment and Support Process
Upon discharge, patients receive a comprehensive assessment to determine their ongoing care needs. This process includes:
- Initial Assessment: Conducted by a multidisciplinary team to evaluate the patient's health, mobility, and social care needs.
- Care Plan Development: A personalised care plan is created based on the assessment findings, outlining the support and services required.
- Ongoing Monitoring: Regular check-ins and assessments to monitor progress and adjust the care plan as needed.
Support Services Provided
Our D2A service offers a range of support services to ensure a smooth transition and effective recovery:
- Personal Care: Assistance with daily living activities such as bathing, dressing, and grooming.
- Medication Management: Support with medication administration and management to ensure adherence to prescribed treatments.
- Therapy Services: Access to physical, occupational, and speech therapy to aid in recovery and rehabilitation.
- Emotional Support: Counseling and mental health support to address emotional and psychological needs.
- Home Adaptations: Assessment and implementation of necessary home modifications to ensure safety and accessibility.
Reablement and Rehabilitation
Our D2A service emphasises reablement and rehabilitation, helping patients regain their independence and improve their quality of life. This includes:
- Reablement Programs: Short-term, intensive support focused on helping patients regain skills and confidence in daily activities.
